1.) First things first, locate the survey that you would like to export. In my example I will be exporting a survey from my "Reservoir Check" form. Select the desired survey as shown below.

2.) You may now chose to export it as a CSV/Excel file or a CSV/Excel (template).

3.) The difference between the 4 options you have are as seen below, first two images are CSV/Excel files containing the survey identifier number, and the two questions in the survey. If you export it as a CSV/Excel (template) as seen in the last two images, you will get the template of the survey showing you all the question types.
