Create a Custom Select Question and Display It as Checkboxes

This SOP explains how to build a questionnaire with a custom select question and change its display style between checkboxes and button-style options. The goal is to help a team member create, test, and compare different select configurations in the survey builder.

Key Steps

1. Create a new questionnaire/template 0:19

  • Open the template or questionnaire builder.

  • Create a new template/questionnaire for testing.

  • Give the template a clear name so it is easy to identify later.

    • Example: test the selects.

2. Add the first question as a text field 0:29

  • Enter a simple text question to collect a respondent’s name.

    • Example: Your name

  • Set the question type to Text.

3. Add a custom select question with answer choices 0:38

  • Add a second question to the template.

  • Write a question that will use selectable answer options.

    • Example: What’s your favorite color?

  • Set the question type to Select.

  • Choose Custom Select as the select style.

  • Enter the answer choices.

  • Save the template after adding the questions and answer choices.

  • Open the survey from the sidebar or survey list.

  • Review how the custom select appears by default.

  • Confirm the options display correctly before making style changes.

5. Change the select question to display as checkboxes 1:29

  • Return to the template builder.

  • Open the settings for the custom select question.

  • Locate the display options at the bottom of the question settings.

  • Change the setting to Show as checkboxes.

  • Save the updated question configuration.

  • Go to the survey results or survey list area.

  • Open the survey again or add a new survey entry.

  • Verify that the custom select now appears as checkboxes instead of standard buttons.

  • Confirm the visual change matches the selected setting.

  • Make a copy of the existing questionnaire or question setup.

  • Rename the copy with a unique name so it does not conflict with the original.

  • Change the display setting on the copied version to Force buttons vertical.

  • Save the copied version.

  • Open the survey again and review the copied version.

  • Compare the appearance of the custom select in both configurations.

  • Confirm the difference between:

    • Checkboxes

    • Button-style options

  • Use this comparison to choose the best display format for the survey purpose.

Cautionary Notes

  • Ensure each copied template or question has a unique name to avoid save conflicts.

  • Save changes after every configuration update so the display settings are not lost.

  • Verify the question type is set correctly before testing the survey, since the display options depend on the select question configuration.

  • If the survey does not display as expected, return to the builder and confirm the custom select setting was applied to the correct question.

Tips for Efficiency

  • Use short, clear test names when building sample questionnaires.

  • Keep answer choices simple during testing so visual differences are easy to spot.

  • Create one version for checkboxes and one version for button display to compare them side by side.

  • Test the survey immediately after saving to catch configuration issues early.

  • Reuse the same sample question text when comparing layouts so only the display style changes.
