How to Embed a Video in a Survey

Embed a video in a survey to help with instructions

1. Open the template editor and create a new survey template 0:20

  • Go to the template editor.

  • Click to create a new template.

  • Give the template a clear name, such as "Survey with Video Instruction".

2. Add the survey question(s) you want respondents to answer 0:31

  • Add a question to the template.

  • Example question used in the demo: "Your name".

  • Build out any other survey fields you want alongside the video.

3. Insert an embedded video into the template 0:49

  • Click Add Embedded Video.

  • Copy a video URL from a source like YouTube.

  • Paste the URL into the embedded video link field.

4. Save the template and verify the video appears in the survey 1:05

  • Click Save.

  • Search for the template you created, such as Embedded Video with Instruction.

  • Open the template to confirm the video is embedded and playable within the survey.

5. Use the embedded video to guide respondents 1:25

  • Share the survey with users who need instructions.

  • Use the embedded video to:

    • explain how to complete the survey,

    • provide context or guidance,

    • support any other instructional purpose.
